No student may participate without completion of their health forms in Magnus. Please pay special attention to completing:

- Athletics Form

- Athletic Travel Consent

- Interval Health History (must be completed within the 30 days prior to the start of the season - AFTER 7/20/2024. If you completed this form in the past, it will need to be completed again).

- Physical Exam Form (exam date within the last 12 months)
